Treating a Lip Attachment That Affects the Gum Tissue
A labial frenum is a band of tissue connecting the inside of the upper or lower lip to the gum tissue. In some patients, its position or the tension it places on the gumline contributes to periodontal, orthodontic, or restorative problems.
Dr. Eleni Kanasi performs labial frenectomy procedures involving the upper or lower lip, most often when the attachment is affecting the health or position of the gum tissue.
Why a Labial Frenectomy May Be Recommended
- A frenum attachment that pulls on the gum margin
- Gum recession, or difficulty keeping the gum tissue healthy in that area
- Orthodontic treatment involving a persistent space between the front teeth
- Interference with restorative or denture treatment
- A prominent or restrictive attachment contributing to local tissue problems

The Periodontal Perspective
When a labial frenum pulls on the gumline, it can make an area harder to clean and may contribute to recession over time. Dr. Kanasi evaluates whether releasing the frenum alone is enough, or whether soft-tissue grafting should be considered at the same time to rebuild tissue that has already been lost.

Coordination with Other Dental Care
The timing depends on why the procedure is being recommended. For orthodontic patients, Dr. Kanasi coordinates with the orthodontist. For periodontal or restorative concerns, treatment is coordinated with your general dentist or restorative provider.
Dr. Amber Hurley Johnson, our oral and maxillofacial surgeon, also performs labial frenectomy and performs lingual frenectomy for tongue-tie, which Dr. Kanasi does not treat.
What Happens During the Procedure
After the area is numbed with local anesthesia, the restrictive attachment is surgically released, and the tissue is repositioned as needed. Sutures may be placed depending on the location and extent of treatment.
Recovery
Mild soreness, swelling, or tightness can occur during early healing. Dr. Kanasi will provide instructions for oral hygiene, diet, and care of the surgical site.
